Do you have pre-diabetes?
Take the online assessment.
People are more likely to have prediabetes and type 2 diabetes if they:
Are 45 years of age or older
Have a family history of type 2 diabetes
Are physically active fewer than three times per week
Have been diagnosed with gestational diabetes during pregnancy or gave birth to a baby weighing more than 9 pounds.
The PreventT2 Diabetes Prevention program is based on research that showed people with prediabetes who lost 5 to 7 percent of their body weight by making modest changes, also reduced their risk of developing Type 2 diabetes by 58 percent. Small changes can add up to a big difference. Working with a trained lifestyle coach who provides guidance, PreventT2 program participants can make lasting changes together.

What is PreventT2 – Our Type 2 Diabetes Prevention Program?
Join Other Participants in Learning How to Make Healthy Changes and Feel Your Best.
1 in 3 American adults have prediabetes, who may develop type 2 diabetes within 3 years if they do not take steps to prevent it. We are offering a proven approach to preventing or delaying the onset of type 2 diabetes through modest lifestyle changes made with the support of a coach and your peers. Community members are preventing type 2 diabetes together with the lifestyle change intervention offered by Sansum Diabetes Research Institute and the YMCA. Guided by trained lifestyle coaches, groups of participants will learn the skills necessary to make lasting changes such as weight loss, being more physically active, and managing stress.
Program Goals: Our mission is to help participants achieve moderate weight loss by eating well and being active. The goal is for participants to lose at least 5-7% of their starting weight and get at least 150 minutes of moderate physical activity each week.

Support from others like you as you learn new skills Join the PreventT2 program — so you can keep doing the things you love.
This year-long program promotes a collaborative, non-judgmental approach to wellness in a motivating environment. Participants will learn how to eat healthily, add physical activity to their routine, manage stress, stay motivated, and solve problems that can get in the way of making changes. Groups will meet once a week for four months, then once a month for the remainder of the program to maintain healthy lifestyle changes. The group setting provides a supportive environment with people who are facing similar challenges and trying to make changes. Together participants celebrate their successes and find ways to overcome obstacles.
